Patient Scheduler

Description
The Patient Scheduler is responsible for scheduling clinical staff for patient visits. Visits will include home, nursing home, board and care facilities and assisted living facilities. In this role, the Patient Scheduler will work closely with the Manager of Population Health, the Director, Clinical Services and the Medical Director to ensure appropriate patient care and clinician capacity needs are met

  1. Actively serve as the point of contact for new and reoccurring patient appointments for internal clinical teams
  2. Participate in process improvement activities by openly sharing thoughts, ideas and constructive feedback to management team
  3. Utilize internal scheduling systems to assist patients and clinicians in appropriately scheduling appointments to ensure all appointments are scheduled as expeditiously and accurately as possible
  4. Actively schedule patient visits that ensure daily clinician capacity demands meet clinician capacity
  5. Professionally answer incoming phone calls and schedule patient visits as appropriate
  6. Monitor, coordinate and reschedule missed visits or canceled visits
  7. Contact patients and clinicians to schedule and confirm visits
  8. Update Manager of changes, cancelations or no shows as appropriate

    • Excel (Basic)
    • CRM (Client Relations Management) Programs Sage ACT!/SalesForce
  • Thrives in fast paced work environment
  • Bilingual in Spanish a plus

Compensation
$17.00.hr
