CLIENT COMPANY OVERVIEW
Our client is a nonprofit organization dedicated to providing targeted, interdisciplinary services for the most vulnerable homeless clients to break or prevent the cycle of homelessness. Our clients Foundation pioneered an innovative and cost-effective solution to advance health and housing stability for the chronically homeless community. To date, our clients housing programs have served 2,316 families and have provided medical and social services to over 20,000 individuals.

POSITION SUMMARY
Medical Coordinator – LVN
The Medical, Outreach and Housing Coordinator is responsible for insuring the discharge and oversight of medical care for homeless clients from hospitals or other agencies contracted with Recuperative Care Center are being properly completed to the standards of Illumination Foundation Managers/ Supervisors/ Directors/ Board of Directors.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Communicate customer needs to management.
  • Work in alignment with the company's mission and values: integrity, ownership, passion, pride, respect, positive energy and drive toward goal attainment.
  • Attain the goals as set forth by management. Seek and follow management coaching toward increased productivity.
  • Complete medical checks
  • Answer phone calls.
  • Maintain clean work and living areas.
  • Initial assessment of emergency situations.
  • Report emergency situations to Managers/Supervisors.
  • Completion initial medical assessment
  • Ensure client has a successful Hospital discharge.
  • Ensure client receives appropriate medical equipment to assist client (i.e. Walker, cane, crutches, wheelchair)
  • Complete the linking of client's with Medicare or Medi-Cal when necessary
  • Verify Follow-up appointments and/or schedule appointments.
  • Home health Coordination.
  • Complete clients exit's
  • Maintain client compliance to program rules and regulations.
  • Maintain clean work and living areas.
  • Transport client to and from appointments as needed.
  • Assist with providing food during meal times as needed.
  • Preparation of Client's living arrangement.
  • Advocate for client to insure best medical care can be provided.
  • Medical Meeting with clients weekly.
  • Establish and maintain a care plan with client.
  • Maintaining client's file.
  • Client Tracking.
  • Medical Notes.
  • Complete client Progress, Extension and Exits reports.
  • Monthly Stories.
  • Attend all IF staff, Recuperative meetings and trainings as requested.
  • Maintain proper inventory of medical equipment and supplies for site.
  • Phone conferences with Health Care Providers.
  • Collaborate with service providers to develop appropriate programs for recuperative clients.
  • Ensure recuperative clients participates and engages in IF social activities.
  • Re-assess safety and liability guidelines to ensure that IF is protected during Recuperative program execution

QUALIFICATIONS / REQUIREMENTS

  • High School Diploma or equivalent.
  • Current non-restricted, LVN license.
  • Current BLS/CPR Card
  • Associates degree preferred
  • 1 year relevant experience
  • Customer and client management
  • Strong self-management skills
  • Be able to make decisions, meet targets and work under pressure.
  • Knowledge of medical terminology, diagnosis and medication.
  • Strong communication and negotiating skills; ability to build and maintain strong patient relationships.
  • Bilingual preferred.

LOCATION/SHIFT REQUIREMENTS

Must be available for assignment to any of the following locations:
(Los Angeles/Lincoln Park), (Alhambra), (Santa Fe Springs) and
(Midway City/Westminster)

Must be available to work a minimum of (1) of the (3) following shifts:
(10:00 am – 7:00 pm) or (2:30 pm – 11:00 pm) or (11:00 pm – 7:00 am)

Commensurate with Experience
